When Should I File a Claim

In the event a registered warranty holder of PermaPlate Windshield Protection experiences damage to their vehicle's windshield from road debris as described in the warranty, a claim should be initiated within 14 days from the onset or discovery of the damage. File a claim using either of the methods described below.

• • A copy of your warranty displaying your Warranty Number
• • Detailed information about the vehicle (i.e. year, make, model, etc.)
• • Date of occurrence of damage
• • Description of the cause of damage
• • Description of the location of damage
